Payment for Services: You agree to pay Dolled Up Dogs for the services provided to your dog at the rates in effect at the start of each visit (Charges). It is your obligation to review the rate of services to be provided to your dog before agreeing to have the services provided and leaving your dog with us. Prices are subject to change without notice and seasonal rates may apply. You are responsible for paying your bill in full at the end of the service. Happy cuts reserves the right to collect any unpaid balance, including but not limited to transferring responsibility for any unpaid balance to a debt collection agency. 

Pre-Existing Conditions: You agree that Dolled Up Dogs is not responsible for any known or un-known pre-existing or degenerative medical condition(s) (or the aggravation of such conditions) including, but not limited to, heart disease, arthritis, intervertebral disc disease (IVDD), cruciate ligament tears, brachycephalic syndrome, or infections that may be affected by the grooming process. You understand that shaving a matted dog may reveal pre-existing skin conditions that could not be identified prior to shaving and that Dolled Up Dogs is not responsible for issues revealed by or that arise due to shaving a matted dog including skin irritations, poor hair re-growth, coat color changes, nicks to moles or skin growths, aural hematomas, and odd behaviors. IN the event mats are found, you authorize Dolled Up Dogs the shave the matted areas. 

Veterinary Treatment: In the unlikely event your dog becomes ill, injured or requires medical attention, Dolled Up Dogs will attempt to notify you or your agent at the phone number(s) you provided. If we cannot reach you or your Agent, or in the case of emergency, you authorize Dolled Up Dogs to serve as your Agent and agree that Dolled Up Dogs may seek veterinary care and make healthcare decisions at your expense. You hereby knowingly, and with informed consent, authorize the release of all veterinary records to Happy Cuts for any medical treatment received by your dog in possible connection with the service. 

Vaccinations: Dolled Up Dogs requires proof that your dog’s Rabies vaccine and any other vaccines required by local or state law is current. Happy Cuts strongly recommends that your dog be vaccinated against infectious conditions, including but not limited to DHPP, Leptospirosis, Bordetella, and Canine Influenza. Vaccinated dogs may still become ill with an infectious condition. You agree that Dolled Up Dogs is not liable if your dog becomes ill with an infectious condition during or after service.

Allergic/Adverse Reactions: Dolled Up Dogs is not responsible for allergic reactions resulting from manufacturer-recommended usage of products. A dog may experience an allergic reaction to grooming products at any time and flea and tick products are associated with a high incidence of reactions. Consult your veterinarian before having your dog treated if you have questions concerning your dog’s sensitivity to such treatments. IF FRONTLINE PLUS HAS BEEN APPLIED, AVOID CONTACT WITH THE TREATED AREA UNTIL DRY. DO NOT ALLOW THE PRODUCT TO BE INGESTED. 

Service Policies, Drying, Express Groom Service: Your dog will be dried using a kennel or velocity dryer (Kennel dryers not used on brachycephalic breeds or senior dogs). If you prefer a specific drying method, it is your responsibility to tell us in advance. Your dog will be tethered during the service. If we feel safety or well being of your dog or groomer/associate is at risk, a muzzle may be used, or we may refuse or discontinue the service. Our preference is not to muzzle any dog. Express Grooming is available for an additional charge if you feel your dog would benefit for any reason including but not limited to, pre-existing medical conditions, breed related conformation, or sensitivity to stress, noise, or kenneling. This requires the dog to be picked up at service completion. The following breeds are REQUIRED to have an Express Groom: English Bulldogs, French Bulldogs, Boston Terriers, Pugs, and Boxers. 

Customer Information/Indemnification: In the event your dog is involved in an incident (e.g. biting a person or another pet) (“Incident”), you authorize Dolled Up Dogs to release your name and contact information to any party involved in the incident and/or governmental authorities. You agree to defend and indemnify Dolled Up Dogs for any claims, damages, or costs arising out of or related to any incident including but not limited to, providing inaccurate information regarding any medical conditions, aggressiveness, vaccinations, or conditions resulting from anxiety that your dog might experience. 

Limitation of Liability: Except as not allowed by law, Dolled Up Dogs, is not liable to you or any other person for indirect, incidental, consequential, special, exemplary, or punitive damages for any matter arising out of or relating to the services performed under this agreement, whether such liability is asserted based on contract, tort, or otherwise. 

Arbitration: You and Dolled Up Dogs agree that except for statutory or common law claims related to intellectual property and disputes that qualify for small claims court, all disputes between us will be resolved by mandatory binding arbitration provisions and procedures. all arbitration claims shall be subject to the laws of the State of Colorado. 

Photo Release: We may take a photo of your dog while your dog is receiving services (“Photo”) You hereby grant Dolled Up Dogs and its affiliates the perpetual, irrevocable, royalty-free right and license to publish, distribute, adapt, modify, or otherwise use the Photo, or any portion thereof, in any manner for any commercial purpose without notice to you/your review or approval. 

Permission for Dogs to Interact: Dolled Up Dogs has permission to allow your dog(s) to sniff, play, or otherwise interact with other dogs on the salon premises. Dolled Up Dogs is not responsible for any injury or damages to you or your dog as a result of you dog being able to sniff, play or otherwise interact with other dogs. 

Lack of Liability for Injury or Incidents that May Result from Entering Dolled Up Dogs: Dolled Up Dogs is not liable for any injuries or incidents that may occur on the premises. Dolled Up Dogs is not responsible for any damages that may occur as a result of you entering the premises.

Agreement: If filling out this service card online, you attest that you or your agent leaving your dog(s) for services is an adult, 18 years of age or older, you agree that clicking the “I Accept” button acts as you electronic signature with the same effect as an original signature and will constitute your acceptance of the terms and conditions herein. You agree that ALL INFORMATION YOU PROVIDED IS CORRECT AND COMPLETE AND THAT IT IS YOUR RESPONSIBILITY TO ADVISE DOLLED UP DOGS OF ANY CHANGES TO YOUR RESPONSE AT THE TIME OF CHECK-IN. Agent: your agent must be an adult, over the age of 18, and not the primary Pet Parent(s). If we cannot reach you, you authorize Dolled Up Dogs to contact your agent and your agent shall have the right to pick up your Dog and have the authority to make all decisions for your Dog, including health-related decisions, and approve the expenditure of funds for, or on the behalf of your Dog.
